WRITTEN NOTARIZED CONSENT FOR BODY PIERCING OF A MINOR CHILD 

  Download, print, fill out, and notarize the Florida Department of Health Written Notarized Consent for Piercing a Minor  

 If you are under 18, your parent or legal guardian must accompany you for the entire  procedure.

 All Minor Consent Forms MUST be notarized BEFORE coming in to get pierced.  

WRITTEN NOTARIZED CONSENT FOR TATTOOING OF A MINOR CHILD, AGE 16 THROUGH 17 YEARS OLD 

If you are 16 or 17 years old and wish to get a tattoo, this is what you will need:
